For court and probation officers, employers, and agencies to verify completion of community-service hours.

How certificate verification works

Every certificate from The Foundation of Change carries a unique verification code. Enter the participant’s first and last name and the code printed on the certificate to see the record we hold: hours completed and hours required, the program status, and the start, issue and completion dates.

When a program is complete, the record also links to the certificate, the hour log and the detailed activity report, so you can print or save them for your file. Checking a record does not require an account or a call to our office. For an overview of the program, read our letter of introduction.

If you cannot locate a record or the portal is unavailable, email info@thefoundationofchange.org. Typical response time: within 1 business day (M–F, 9am–6pm ET). For urgent court dates, include defendant name, DOB, case #, and verification code.
